A New Health/Dental Insurance Benefit Policy is in effect since 5/1/09. Three options are offered: (1) A Base Plan - Health Savings Plan, (2) Option 1– an HMO Plan and (3) Option 2 - an HMO with Opt-out Open Access. Please refer to the details of these plans prepared by the Company's insurance Representative, The Jordan Insurance Group. The document may be accessed via: http://panum.com/intranet/Panum_Health_Benefit_Details_2009.pdf Enrollment Forms: To enroll, please select, complete and fax to 301-299-4216 the applicable Panum Health Insurance Application Form for the desired option: · Base Plan – HSA Open Access HMO Blue Choice: http://panum.com/intranet/HMO_HSA_EE_App.pdf · Option 1 – HMO Blue Choice: http://panum.com/intranet/HMO_EE_App.pdf · Option 2 (Plan in effect since 5/1/2008) – Open Access Blue Choice: http://panum.com/intranet/Enrollment_Form.pdf Cost Summary: For Employee only Coverage, · The employee is only responsible for $ 72 per month for the Base Plan cost including Dental. · The Company pays 100% of the deductible ($1,200). · If an employee chooses either Option 1 or Option 2, it will be the responsibility of the employee to pay for the difference in cost. For Spouse and Dependent coverage, · Payment of the coverage cost is the sole responsibility of the employee except: Panum will contribute $1,200/year towards dependents' full deductible if an employee opts for the Base Plan (HSA) or contribute $100 per month towards monthly health/dental insurance premium if an employee chooses Option 1 or Option 2.
